| Supports for Youth at Risk |
 |
READY4LIFE is a pilot project, funded by the United Ways of Oakville and Burlington to support youth at risk in these communities. Four key objectives: |

- Agency outreach - Initiate connections between youth and adult services to assist
with service response and enhance planning activities in the transition of youth
from youth to adult mental health services.
- Youth outreach – seek out and identify youth at risk via traditional and non-
traditional approaches.
- Early intervention – for those youth who are not ready to transition to the adult
system, initial intervention will be provided to reconnect them to youth services,
education and their families.
- Intervention support – for those youth who are at risk and wish to transition to
the adult system, a Youth Outreach Coordinator will provide case management
support to assist with a successful transition and integrate them into the
community.
