How to Access Supportive Housing Services

Criteria for Support & Housing – Halton’s services and supportive housing
18 years of age or over
On-going diagnosed mental illness
Documented need for support services
Medication Compliant
Willing to provide a Criminal Reference check
Source of ongoing income (housing only)

If all Criteria is met:
 
Submit completed application form to Support & Housing – Halton

• Applications received, will be prioritized on the agency’s sequential waiting list
• Clients will be advised in writing and provided with some basic telephone/in person support services while on the waiting list.
• When an application is selected, the prospective client will be requested to come to an initial assessment interview
• When a decision has been made about the application, staff will advise the applicant and the referring party of the eligibility and status of the application
• Income Verification and Criminal Reference checks will be required as part of application approval
• In shared housing situations, the prospective tenant will meet with the existing residents to determine suitability
• Clients will complete income verification forms and sign Conditions of Tenancy forms prior to admission into the program
• After a three month satisfactory probation period, tenants will be invited to sign a lease agreement

211 is an easy-to-remember, non-emergency telephone number that connects people to a full range of community services.  Twenty-four hours a day, seven days a week, Certified and Referral Specialists will answer 211 calls, assess the needs of each caller and link them to the best available information and services.
