Youth Supports

Transitional Aged Youth

The TAY Service Coordination service is a Mississauga-Halton LHIN initiative that provides a monthly place where agency staff come together to problem solve and assist youth who may benefit from an enhanced service plan and to successfully bridge youth into adult services.
The goal of the TAY program is to provide an active response within 48 hours of first being contacted

Ready 4 Life

Our Ready4Life program is the pillar of the youth programs, offering community-based support and providing system navigation and advocacy to link youth to needed services and resources. Our case managers are mobile, meeting with youth in their preferred surroundings within the community, rather than requiring youth to attend in-office appointments. They assist at-risk youth in meeting the needs of all areas of their lives and help them get the medical, financial, emotional, employment and recreational supports they need. Contact: 905-699-1401.

Youth Supportive Housing

Supportive and affordable housing is available in a range of housing models, from a shared, or congregate living with medium support with staff available at regular times each day; to low-support housing where youth live independently and manage their homes, practicing the skills of independent living without ongoing staff support.

The Youth Harm Reduction Project

The Youth Harm Reduction Project is a free service for youth between the ages of 15-29 in the Halton Region. This program provides training, street level outreach, information and support groups to encourage safer decision making in reducing the risks associated with drug use, sex, homelessness, violence and discrimination.
